Application Erect Monument

The Application Erect Monument is used for the installation of a monument in the Western Downs Cemeteries. This form is to be completed by a registered stonemason and the customer.

Application for Grave/Niche Reserve

The application for grave/niche reserve is used when a plot within the Western Downs Cemeteries is pre purchased and reserved.

Application for Installation of Memorial Plaque

The application for installation of memorial plaque is to be completed for new plaques, replacement plaques and second/third inscriptions on existing plaques at all the Western Downs Lawn Cemeteries. Please note, this application is not to be used to install a services Memorial Plaque that is showcased on the Dalby Services Memorial Wall.

Application for Interment – Ashes

The application for interment of ashes is to be used to process your request to inter ashes at one of our Western Downs Cemeteries. Council has many options available when it comes to the location of the interment of ashes including columbarium walls, ashes gardens or burial of ashes into a grave.

The application for interment - grave is to only be completed by a funeral director. This form is required to undertake a burial within the Western Downs Region.

Application for Myall Remembrance Park Services Memorial Wall Plaque

The Myall Remembrance Park Services Memorial, or remembrance wall, recognises the Australian military service of members of the community regardless of whether they served in active duty or not. The memorial is a wonderful addition to the cemetery and a fitting tribute to those selfless individuals who were prepared to defend our country.  Application for Myall Remembrance Park Services Memorial Wall Plaque(PDF, 217KB)  are completed once a year to fit in with Anzac Day celebrations. Please note applications can be completed at any time throughout the year and must include Statement of Service and Permission to use Service Badge together with the application and prescribed fee.

Application Refurbish Monument

If you have noticed a monument is faded or possibly hard to read, you may consider getting the monument refurbished. If you are interested in making a monument look new again, an (PDF, 321KB)Application for Refurbishment of Monument(PDF, 245KB)  needs to be completed by a stonemason and person wishing to refurbish monument.
