Community Care

 

Community Care Services are operated and delivered by Western Downs Regional Council with the support of Commonwealth Home Support Program and Aged Care funding. Our motto is ‘Local people assisting local people to stay safe and well in their homes’.

Services are offered from centers in Miles, Chinchilla, Jandowae, Meandarra, Miles and Tara, with some services being offered to a broader area. Our various centers offer selected services including, in home maintenance and care, live in facilities, community support and more. Eligibility criteria apply to all services and our friendly team can assist you, or your family member with information about the criteria and how to register for assessment.

Community Care Services:

Chinchilla Home Maintenance

Chinchilla Home Maintenance provides practical assistance with home maintenance and minor repairs for frail aged clients and younger persons with a disability to support them to remain in their home environment.
Home Maintenance assistance may include basic gardening, lawn mowing, gutter cleaning, rubbish removal and basic home repairs with the focus of improving safety, accessibility and independence.

Eligibility:

The eligibility of clients for Home Maintenance services is assessed by the My Aged Care call centre. Register your request for assessment and a trained assessor will speak to you about your care needs and eligibility for services.

Fees:

Jandowae Domiciliary Care

Jandowae Domiciliary Care has been striving to assist frail aged residents and younger disabled members of our community, and their carers to obtain and maintain their optimal health, wellbeing and independence in their own home since 1993.

The Domiciliary Care Service may offer support including:
In-home and flexible respite care for carers, Personal care assistance (eg hygiene and grooming assistance), Medication management assistance, Social support (eg shopping, banking ans social visits), Domestic duty assistance, Social support Group activities, Transport services for medical appointments, Basic home and garden maintenance assistance and home repair, Meal assistance, Therapy programs established by Allied Health Professionals, Nursing Care, Chronic condition support and other services as negotiated with service providers.

The eligibility of clients for Domiciliary Care services is assessed by the My Aged Care call centre. Register your request for assessment and a trained assessor will speak to you about your care needs and eligibility for services.

Service Area:
Jandowae, Burra Burri, Warra, Cooranga North, Bell, and Jimbour areas.

Meandarra Home Nursing

Sine 1992, the Meandarra Home Nursing Service has provided basic support services to frail aged residents and younger people with a disability to support them to live in their community. These services provide eligible residents with assistance at home, support accessing community services and supporting carers.

The Home Nursing Service may offer support including:
In-home and flexible respite care for carers, Personal care assistance (eg hygiene and grooming assistance), Medication management assistance, Social support (eg shopping, banking ans social visits), Domestic duty assistance, Social support Group activities, Transport services for medical appointments, Basic home and garden maintenance assistance and home repair, Meal assistance, Therapy programs established by Allied Health Professionals, Nursing Care, Chronic condition support and other services as negotiated with service providers.

Meandarra Home Nursing Service also delivers Home Care Packages to clients with more complex needs but wish to stay in their own homes longer.

Miles Community Care

Established in 1997, Miles Community Care delivers Home Care Packages to Miles and surrounding areas.

The Miles Community Care Service may offer support including:

Home Care Packages are for clients with more complex care needs but wish to stay in their homes longer.

Tara Community Care

Tara Community Care aims to enhance the independence of the frail aged residents and people with disabilities by providing services to enable them to remain in their home.
Services are prioritised to those residents who are vulnerable and whose ability to live in the community is dependent on provision of external support.
Tara Community Care also deliver Home Care Packages to clients with more complex needs but wish to stay in their own homes longer.
